To the parents of incoming 7th grade students:
Massachusetts Department of Public Health regulations mandate that your child must have the following before entering seventh grade.

Physical Examination

1 booster dose of Td/Tdap (tetanus-diphtheria-pertussis) within the past 5 years and 1 dose of MenACWY (meningococcal vaccine). Td/ Tdap booster and MenACWY vaccine are usually administered at the sixth grade physical examination.
Please send a copy of your 7th grader's most recent physical exam and immunization record before the start of school.
All Student Health forms can be submitted by email to [email protected], fax (978) 529-2170, by regular mail (502 Cabot St., Beverly MA 01915) or hand delivered to the office.
Please complete this request prior to the start of the 2022-2023 school year.
